PREVENTIVE HEALTH CARE
• Annual Physical Examination (Executive Out-Patient Package)
• Administration of immunizations, excluding cost of vaccine/serum.
• Consultations and advice on diet, exercise and other health habits
• Well-baby care (for member's unenrolled baby less than 3 months of age) at Fortune CARE owned full service clinics.
• Monitoring of health problems
OUT-PATIENT CARE
• Consultations at any Fortune CARE owned clinics in Metro Manila and provincial key cities and all Fortune
CARE accredited facilities nationwide.
• Eyes, Ears, Nose, and Throat Care
• Treatment of lacerations, abrasions, mild burns, sprains and other minor injuries
• Minor surgery procedures available at Fortune CARE Clinics for covered lesions
• Necessary routine laboratory test and commonly available diagnostic procedures, including ECG & X-ray
examinations, as may be prescribed by Fortune CARE affiliated physicians & specialists
• Pre and post-natal care at Fortune CARE-owned clinics.
• Physiotherapy Treatment up to a maximum of 10 sessions per year
IN-PATIENT CARE
• Direct access to Accredited Hospitals including St. Luke's Medical Center, The Medical City, Makati Medical Center, Asian Hospital and Cardinal Santos Medical Center.
• Room and Board: Private-Open
• Professional services of the Fortune CARE attending physician/s
• Drugs and medications needed while confined in the hospital
• Whole blood, packed RBC, platelet concentrate, plasma and intravenous fluids
• Use of operating/ recovery room
• Routine laboratory examinations and commonly available diagnostic procedures as ordered by the Fortune
CARE affiliated physician/s
• Oxygen and its administration
• Dressings, plaster casts, sutures and other miscellaneous supplies directly used in the treatment of the
covered ailment
• Indicated use of ICU, CCU and special units subject to the maximum limits for the underlying dreaded disease
