DENTAL CARE (Optional Benefit)
Available to members at Fortune CARE in-house dentists or at any Fortune CARE accredited dental clinics.
WORLWIDE EMERGENCY CARE
Fortune CARE shall cover eighty percent (80%) of the total approved emergency treatment and hospitalization
charges while in the course of travel incurred by the member when treated in a foreign country not to exceed the
amount of what it could have cost if treatment is done by a FORTUNE CARE affiliated physician in an affiliated
hospital.
THIRD PARTY LIABILITY (TPL) / WORK RELATED ACCIDENTS (ECC) / UNPROVOKED ASSAULT
Fortune CARE may cover Medical and Hospital Services extended to a member for bodily injuries established to
have been caused by any compensable act of a Third Party (Motor Vehicle accidents), work related injuries and
injuries caused by Unprovoked assault up to P 20,000.00.
MATERNITY ASSISTANCE
Fortune CARE shall grant Annual Maximum medical services to any employee or employee's dependent for a
maximum amount of Five Thousand Pesos (P5,000.00) to help defray hospitalization charges for abnormal
pregnancy (ectopic, placenta previa, abruption placenta or post-partum atony) requiring surgical operation.
(Except: Member who is 35yrs. old and above or surgery for any other pregnancy related indication or conditions
not indicated above)
